Job Overview
We are seeking compassionate and dedicated Health Care Aides to provide high-quality patient care in various healthcare settings, including assisted living, nursing homes, and home health environments. The ideal candidate will be responsible for supporting patients with their daily needs, ensuring their safety, comfort, and dignity. Experience with dementia, Alzheimer’s, memory care, and working with individuals with developmental disabilities is highly valued. The role requires strong communication skills, attention to detail, and familiarity with medical documentation and EMR/EHR systems. This position offers an opportunity to make a meaningful difference in patients’ lives while working in a supportive team environment.

Responsibilities

  • Observe and monitor patients’ conditions, reporting any changes or concerns promptly.
  • Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support.
  • Administer medication and perform basic medical tasks following prescribed protocols.
  • Record patient information accurately using EMR/EHR systems while maintaining HIPAA compliance.
  • Support patients with meal preparation and feeding as needed.
  • Assist with catheter care, wound care, and other specialized procedures such as Hoyer lift transfers.
  • Conduct vital signs assessments including blood pressure, pulse, temperature, and respiration.
  • Provide compassionate dementia and Alzheimer’s care tailored to individual needs.
  • Support hospice care patients by providing comfort measures and emotional support.
  • Maintain a clean and safe environment through light housekeeping duties such as laundry and cleaning.
  • Document medical records thoroughly and accurately in accordance with healthcare regulations.
  • Work collaboratively with nursing staff and other healthcare professionals to deliver comprehensive patient care

Qualifications

  • Prior experience in long-term care, home health, or senior care preferred.
  • Knowledge of medical terminology and medical documentation practices.
  • Certification in CPR required; additional certifications such as medication administration or hospice care are a plus.
  • Experience working with people with disabilities or developmental challenges highly desirable.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Compassionate attitude with excellent communication skills.
  • Ability to handle sensitive patient information following HIPAA guidelines.
  • Physical ability to assist with patient mobility including use of Hoyer lifts and other assistive devices.
